Terms And Conditions

These Terms and Conditions explain what you can expect from us and what we expect from you as a member of our Direct Primary Care (DPC) service.

 

1. What We Offer

As a Family DOC DPC member, you'll receive:

 

  • Direct communication with your doctor (text, call, email)

  • Same-day or next-day appointments

  • Extended appointment times

Basic in-office procedures and tests (as listed on our website or welcome packet)

 

We are not an insurance plan. You’ll still need insurance for hospitalizations, specialist visits, or expensive tests not offered in-house (like MRIs or CT scans).

 

2. Membership Fees

Membership is charged monthly (or annually, if you choose).

 

  • Payment is due at the beginning of each billing cycle.

  • Missed payments may result in a pause or cancellation of services.

  • Prices are subject to change, but we will always give you at least 30 days’ notice.

 

3. Cancellations

You may cancel your membership at any time with 30 days’ written notice.

 

We may also cancel your membership with 30 days’ notice if necessary (e.g., repeated missed payments or inappropriate behavior).

 

4. No Insurance Billing

Family DOC DPC does not bill insurance, Medicare, or Medicaid. You are responsible for paying the membership fee directly.

 

5. Privacy

We protect your health information in accordance with HIPAA. We won’t share your personal information without your consent unless legally required.

 

6. Communication

We may communicate with you by phone, text, email, or through a secure patient portal. You acknowledge that electronic communication carries some privacy risks.

 

7. Emergencies

We do not offer emergency or urgent care services. For emergencies, call 911 or go to the nearest emergency room.

 

By enrolling in Family DOC DPC, you agree to these Terms and Conditions.
